Overview

Acne is a skin ailment that plagues a majority of the youth of the modern world. It affects not only the appearance but also the quality of life and confidence of the bearer.

Nowadays, it is a common fact that acne does not only affect teenagers but also adults who may not have had acne at all in the past.

Acne has four majors pathomechanisms –

  • Blockage of skin pores which could be related to genetic makeup and hormonal changes.
  • Excessive sebum production or oiliness further clogs the pores.
  • Acne bacterial overgrowth i.e., Propionibacterium acnes over the skin.

Inflammation due to the reaction of the immune system to attack these bacteria.

Additional common factors that influence the clinical presentation and severity of acne include environmental pollution, disturbance in the microbiome, hygiene, imbalances in our metabolic functions relating to our diet.

Acne can have various presentations depending on its causative factors and severity – blackheads, whiteheads, papules, pustules, nodules, and even cysts. However, acne does not occur alone. Companion morphologies coexist with acne including uneven skin tone, pigmentation, acanthosis nigricans, skin tags, DPNs, abnormal facial hair growth, and scars amongst many. Acne is not only confined to the face but may also affect the back, chest, and scalp.

Acne is also a predictor of metabolic syndrome in the future. Obesity and family history of acne and metabolic diseases such as diabetes mellitus, hypertension, and cardiac disease puts an individual at higher risk of developing acne. Certain medications such as steroids, anti-tubercular treatment, and anti-cancer treatment can also result in acne. Stress, smoking, diet high in sugar and dairy intake can aggravate acne.
